The dirt and sod have been placed up against the trunk of the tree (Volcano Mulching) 
The soil is at least 2' high on this tree trunk. There were other trees in the lawn that had the same thing done to them.

There should be no more than 3 "of mulch around the tree and no mulch should be touching the bark of the tree.
